
CHW PIPELINE
Let’s begin by painting some pictures that aren’t pretty:
1: Mario has been dealing with difficulty breathing for so long that he can’t even remember when the issue began. He’s aware that his living situation is playing a role - he’s no stranger to seeing mold in his apartment complex, and can even tell when the air isn’t safe to breathe. But Mario works full-time and attends classes at night, so he has no time to figure out what can be done to fix his situation.
2: Trisha is fighting tooth and nail to safeguard her two children. The battle has never been easy - her kids experience the effects of their illnesses at a frequency that disrupts her ability to provide. The emergency room visits are one thing - the distance Trisha must travel to receive even basic care is a whole other issue.
3: Jen and Abel are seeking assistance for their asthma, and have heard through various coworkers about managed care plans and Medi-Cal. However, their experience with healthcare up to this point has been far from ideal: even though materials exist in English and Spanish, the volume is overwhelming, and they don’t know where to begin.
These stories are too common. Community members navigating chronic illness with nowhere to turn, left to their resilience as the systems in place widen equity gaps.​
This is where Safer Together’s Community Health Worker (CHW) pipeline comes in.
CHWs who come into the neighborhoods we serve.

CHWs who understand the health landscape and make sure you understand.

CHWs who lead with the heart.

We employ CHWs who conduct outreach, home and field visits, care navigation, health education, and warm handoffs to clinical providers. CHWs build sustained relationships across months and years, accompany patients to appointments, advocate inside clinical settings, and translate (linguistically and culturally) between patients and providers.

And now we’re building out a pipeline to expand this care network further.
The team is rolling up our sleeves to augment the community-first impact we’ve already made and train new CHWs who synthesize skill and passion with intimate familiarity with the community’s needs. Our huge vision is an expansive, citywide network of CHWs who keep our communities living with dignity, agency, and resilience.
