Community Care Health Educator

About the Role

This role supports the Safer Together Programs/Advocacy/Policy team. Under the supervision of the Community Care Programs Manager and Community Care Programs Coordinator, the Community Care Health Educator will primarily focus on high quality and equitable delivery of the Safer Air and Safe Care Programs. Additionally, this role will support all community care program areas in regards to creating, disseminating, organizing and engaging our community members to our services to increase awareness and community capacity to advocate for and utilize public health tools, harm reduction, and community interventions that support their health and wellbeing. This includes facilitating health education presentations, trainings, workshops in our office and externally at partner sites, designing and promoting educational materials about services and relevant resources, building and distributing air filters and air monitors, collaborating with Safer Air partners, attending Safer Air coalition meetings, supporting CLIA waived testing and care navigation.

 

Essential Duties and Responsibilities

 

General Responsibilities:

  • Develop educational and promotional materials for use in facilitating culturally and developmentally appropriate health education and promotion in community settings and partner sites.

  • Meet work obligations through efficient work habits such as: Meeting deadlines, honoring schedules, thorough communication, attending and actively participating in staff and individual meetings.

  • Support the programs team in implementation and execution of various community care projects and goals through day-to-day tasks.

  • Support collection of performance metrics, monitor, and analyze data to be used for continuous improvement ideation and execution. 

  • Perform other related work duties as needed or as required by supervisor and programs/advocacy/policy teams.

  • Create supportive environments where community members and participants feel comfortable discussing sensitive topics in 1:1 or group settings.

  • Participate in outreach and in-reach activities, in person, virtually, via social media, as appropriate.

  • Attend relevant community and coalition meetings and trainings.

  • Enter data in google sheets or future CRM accurately and promptly.

  • Use platforms such as Canva to build out training presentations, fliers, and infographics with the team.

  • Support the training and professional development of community members and Safer Together staff about health education resources, curricula, projects, materials, and goals. 

  • Deliver services and track completion of supported referrals and document status/progress in various platforms.

  • Complete all necessary paperwork, health record documentation, and reporting requirements in a timely fashion.

 

Safer Air (30%):

  • Assist community members in informed decision-making around Safer Air, clean air and harmful air mitigation by providing accurate and comprehensive information both orally and written.

  • Support community capacity building through collaborations with partners to advance youth-led environmental justice initiatives related to Safer Air

  • Conduct quarterly community convasses of Safer Air household and community care participants through virtual, in person or phone meetings.

  • Facilitate community deployment of air monitors and filtration devices

  • Maintain up-to-date knowledge in the fields of clean air, environmental justice, asthma and respiratory illnesses, wildfires, harm reduction, health education, positive youth development, and client-centered care.

  • Attend and receive certification through the CA DPH Asthma Management Academy or equivalent training.

Safer Care (50%):

  • Disseminate our various programs’ educational materials and care tools such as harm reduction, Naloxone, rapid antigen COVID-19 and Hep C tests and other CLIA waived tests.

  • Conduct on and off site training on overdose prevention and Naloxone distribution to anyone who may be at risk of experiencing or witnessing an opioid overdose, including people who use drugs (opioids or otherwise), their family, members of their social networks and service providers. 

  • Provide written and verbal harm reduction information on overdose prevention, safer injection strategies, and Nasal/intramuscular Naloxone administration.

  • Participate in street outreach to engage community members through active listening and referral to providers of evidence based supports to improve health outcomes as well as medicine for opioid use disorder.

  • Proactively work to develop ongoing client relationships across the program’s varied interventions, through consistent and compassionate support of individuals strengths and challenges. 

  • Provide individual and group health education and organizational presentations on harm reduction principles and practices as a public health intervention, particularly focused on areas of safer sex, prevention and treatment of infectious diseases, safer injection drug use interventions, safer use supplies, and overdose reversal training.

  • Support community distribution through street outreach, community events and partnerships with community based organizations

  • Community Outreach, Public Health Messaging, Peer Support and Care Navigation (20%):

  • Support peer based support group events, including set up/break down of meeting room, preparation of facilitation materials and follow up with care navigation and support resources.

  • Guide and support uninsured community members on how to sign up for Medi-Cal and other insurance providers that support CalAim’s “community supports” provided by Safer Together.

  • Support CCP team with home visits to deliver “community supports” including asthma remediation, asthma preventative services and support with adherence to clinical care plans

  • Provide clients with easily accessible and low threshold services that operate within a harm reduction framework using referral systems.

  • Conduct outreach, base building, training, education and leadership development among our participants and community members.

 

Experience, Qualifications, and Skills

  • Minimum of one year direct experience serving populations of people with substance use disorders, or equivalent lived experience 

  • Driver’s license and ability to transport programmatic supplies 

  • Bilingual in Chinese preferred 

  • Cultural and socioeconomic sensitivity in dealing with individuals and staff of diverse backgrounds

  • Interest in racial equity, environmental justice, public health, mental health, and well-being, particularly related to youth and families

  • Experience providing culturally relevant human support such as direct services and/or community organizing with target populations (BIPOC: Black Indigenous People of Color, PWUD: People Who Use Drugs, or unhoused community) particularly harm reduction and care navigation services and/or other experience serving these target populations 

  • Familiarity with and commitment to Harm Reduction theories

  • Experiencing coordinating logistics that involve inventory, storage, supplies distribution, etc. are preferred

  • Experience training, facilitation, presenting and supervising individuals including volunteers are preferred

  • Must be very comfortable working in “street-based” outreach situations, community events, residences (e.g., SROs) with marginalized populations.

  • Self-directed, motivated, and flexible with the ability to work independently and as part of a team

  • Willingness to learn, give and receive feedback, supportive of creative environments that encourage knowledge sharing

  • Strong organizational, interpersonal and communication (written and verbal) skills required

  • Fully vaccinated against Covid-19 

  • Livescan background check

  • Tuberculosis Test

  • Ability to be flexible and work business hours with some evenings, early mornings, and weekends required.

 

Preferred Qualifications

  • 5+ years of progressive leadership experience in fundraising, donor relations, or development within a non-profit organization.

  • Thoroughly understands the funding landscape and opportunities, particularly for health equity and environmental justice organizations and has the relationships to a diverse group of funders.

  • Strong interpersonal skills with the ability to build and maintain relationships with diverse stakeholders, including donors, volunteers, and staff.

  • Excellent written and verbal communication skills, including grant writing and storytelling abilities.

  • Ability to think strategically, develop creative solutions, and execute plans to achieve fundraising goals.

  • Proficiency in using donor management software or fundraising databases to track donor interactions and manage campaigns.

  • Knowledge of fundraising strategies and techniques, including major gifts, corporate sponsorships, grants, and online giving platforms.

  • Proficiency in financial statements, developing, forecasting, and managing budgets.

  • Familiarity with the non-profit sector, including trends, best practices, and ethical considerations.

  • Strong organizational skills with the ability to manage multiple priorities and meet deadlines.

  • Passion for the mission, vision, and values of Safer Together.

 

Physical Requirements

  • The ability to lift and transfer materials up to 20 lbs. is required.

  • Ability to stand and walk for extended periods of time are required.

Supervisory Responsibilities

  • This role does not have direct reports but may possibly oversee future volunteers.

 

Schedule

  • This is an hourly position, requiring a commitment of 30 hours per week. Work hours are generally within our operating hours of Monday - Friday from 9 am - 5:30 pm on weekdays, and each shift will be no more than 8 hours per day. Some evenings, early mornings, and weekend work may be required. This position requires onsite presence in the community and at Safer Together office at 29 Precita Ave. in San Francisco, CA. Up to 6 hours (20%) of remote work is available weekly.
     

Compensation and Benefits

  • Compensation is $26/hour, benchmarked to comparable roles in comparable organizations in San Francisco. Subsidized health insurance (medical, dental, vision), 1% 401K retirement matching, and 3 weeks paid time off benefits.
